Use this screen to link required medical tests with an employee. This table serves as a log of medical tests for each employee. For example, your company may require tuberculin testing or a pre-employment physical.
You enter medical test information on this screen as the information becomes available. Before entering information on this screen, you must first set up medical test codes on the Manage Medical Surveillance Codes screen. You can add to this screen at any time, and enter an unlimited number of records.
Enter, or click to select, the employee ID in this field. The employee name displays in field to the right. Costpoint validates your entry against entries made on the Manage Employee Information screen.
Enter, or click to select, the code for the medical test that applies to this employee. Costpoint validates the code against the Medical Surveillance table in Costpoint Personnel. The medical test description displays in the field to the right.
Select the code that indicates the frequency of this test. Valid values are:
Daily
Weekly
Monthly
Semi-Annual
Annual
Other
If the Results field is set to Pending in the Details table, this non-editable field displays the next due date for this test, as specified in the Next Due Date field on the Details table.
Enter any comments you want to record for this row.
Use this table to enter additional information about this medical test for this employee.

Click on the toolbar. Then enter, or click
to select, the date, in MM/DD/YYYY format, on which this medical test is next due for this employee. If you have already entered rows in this table, the date defaults into this field, based on the Frequency you selected. Costpoint calculates the next due dates as follows:
Daily — Last date + 1 day
Weekly — Last date + 7 days
Monthly — Last date + 1 month
Semi-Annual — Last date + 6 months
Annual — Last date + 1 year
Other — No default
If the due date is earlier than the system date, Costpoint displays a warning message. You can override the warning message if the date is the one you want.
Enter, or click to select, the date, in MM/DD/YYYY format, on which this test was performed.
If you enter a date in this field, the Results field must be Pass or Fail.
If you set the Results field to Waived or Pending, you cannot enter a date in this field.
Select the result of this test from the list of values available in the drop-down list. Valid values are:
Pass — The employee passed the test.
Fail — The employee did not pass the test.
Waived — The employee voluntarily waived this test.
Pending — The test has not yet been performed.
This option works in conjunction with the Test Date option:
If you set this option to Pass or Fail, you must specify a date for the Test Date field.
If you set this option to Waived or Pending, you cannot enter a date in the Test Date field.
Only one row in the table can have a results status of Pending.
Name or describe the medical facility where this test was performed.
Enter the name of the physician who administered or performed this test.
Enter any comments you want to record for this row.
