Select this check box to indicate that the employee is to be reported as an Other Protected Veteran in the VETS-4212 report. The U.S. Department of Labor defines “Active Duty Wartime or Campaign Badge Veteran” as a veteran who served on active duty in the U.S. military, ground, naval or air service during a war or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense.
Select this check box to indicate that the employee is an Armed Forces Service Medal Veteran. An Armed Forces Service Medal Veteran is a veteran who, while serving on active duty in the U.S. military, ground, naval or air service, participated in a United States military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985 (61 FR 1209).
Select this check box to indicate that the employee is a disabled veteran. A disabled veteran is:
A veteran of the U.S. military, ground, naval, or air service who is entitled to compensation (or who but for the receipt of military retired pay would be entitled to compensation) under laws administered by the Secretary of Veterans Affairs;
A person who was discharged or released from active duty because of a services-connected disability.
Select this check box if the employee is a veteran who belongs to at least one of following veterans but opted not to self-identify a specific category:

If applicable, enter the date (in MM/DD/YYYY format) on which this employee was discharged or released from active military duty. This date is used to determine the Recently Separated Veterans to be included in the count of Protected Veterans in the VETS-4212 report.
A Recently Separated Veteran in VETS-4212 report is a veteran during the three-year period beginning on the date of such veteran’s discharge or release from active duty in the U.S. military, ground, naval or air service.
Select this check box to indicate that the employee is a Special Disabled Veteran. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing. Previously, this information was used in VETS-100 report.
The following determines Special Disabled Veteran status:
The veteran is a veteran of the U.S. military, ground, naval or air service who is entitled to compensation under laws administered by the Department of Veterans' Affairs for a disability unless he is receiving military retired pay.
The disability is rated at 30 percent or more.
The disability is rated at 10 or 20 percent in the case of a veteran who has been determined under Section 38 U.S.C. 3106 to have a serious employment handicap.
The veteran was discharged or released from active duty because of a service-connected disability.
Select this check box to indicate that the employee is a Vietnam Era Veteran. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing. Previously, this information was used in VETS-100 report.
The following determines Vietnam Era Veteran status:
The veteran served on active duty in the U.S. military, ground, naval, or air service for a period of more than 180 days and was discharged or released with other than a dishonorable discharge, if any part of that active duty was performed:
In the Republic of Vietnam between February 28, 1961, and May 7, 1975.
Between August 5, 1964, and May 7, 1975, in all other cases.
The veteran was discharged or released from active duty in the U.S. military, ground, naval, or air service for a service-connected disability, if any part of that active duty was performed:
