Use this screen to design a customized version of a review form or forms. The review form is divided into five sections, each of which you can include or exclude, depending on your company's requirements.
Use this screen to set up review forms as part of your system initialization. You must set up review forms before you can assign a review code to an employee on the Manage Employee Salary Information screen.
Use the fields in this group box to uniquely identify a review form.
Enter a unique code to identify this review form.
Enter a description for the Review Form ID you have assigned. This is a required field.
Use the options in this group box to select information from the Manage Employee Information and Manage Employee Salary Information screens to be included on your form.
Use the options in this group box to indicate which values found on the Manage Employee Information screen you want to include on the review form.
Select this check box if you want the employee's social security number to display in Section I of the review form.
Select this check box if you want the employee's hire date to display in Section I of the review form.
Select this check box if you want the employee's last review date to display in Section I of the review form.
Select this check box if you want the employee's supervisor's name to display in Section I of the review form.
Use the options in this group box to indicate which values found on the Manage Employee Salary Information screen you want to include on the review form.
Select this check box if you want the employee's compensation plan to display in Section I of the review form.
Select this check box if you want the employee's grade to display in Section I of the review form.
Select this option if you want the employee's step to display in Section I of the review form.
Select this option if you want the code for Hourly or Salary to display in Section I of the review form.
Select this option if you want the job title to display in Section I of the review form.
Select this option if want the current hourly rate to display in Section I of the review form.
Select this option if want the current salary rate to display in Section I of the review form.
Select this option if you want the current annual rate to display in Section I of the review form.
Select this option if you want the last percent increase value to display in Section I of the review form.
Select this option if you want the current rating value to display in Section I of the review form.
Select this option if you want the labor group or union to which this employee is assigned to display in Section I of the review form.
Select this option if you want the location or union local to which this employee is assigned to display in Section I of the review form.
Select this option if you want the General Labor Category (GLC) to which this employee is assigned to display in Section I of the review form.
Select this option if you want the organization to which this employee is assigned to display in Section I of the review form.
Select this option if you want the employee's manager's name to display in Section I of the review form.
